Program Requirements

The Radiation Oncology Accreditation Program Requirements explain the overall process and requirements for application. We recommend that you print out and carefully read the Program Requirements before beginning the application process.

Application Forms and Instructions

The application itself is a two-part document:

  • Part I – Sections A through H must be completed for each site (facility) in the practice applying for accreditation and includes the name, location and type of site (hospital or free standing), services offered, patient census data, equipment lists and devices, staffing forms, and machine calibration data forms.
  • Part II – Sections A through D apply to the practice and include policies and procedures, QA activities, physician release and attestation, physicist attestation, practice self-assessment, and compliance with the ACR Practice Guidelines and Technical Standards.

Once you have read the Program Requirements, proceed by printing out the application checklist, the application Parts I and II, then proceed to the Survey Agreement, HIPAA/Business Associate Agreement, Census Data Form, and sample data collection form for preliminary case self-assessment. Complete the process by referencing the checklist and verifying that all documents have been printed. This checklist can then be used as you proceed through the application process.
