Resources to assist facilities on establishing just culture, using peer-learning as an expression of just culture and implementing best practices.
The effectiveness of a radiology quality and patient safety program is improved by understanding and addressing errors, but errors must first be disclosed for this to happen. Staff reluctance to report mistakes due to fear of punishment can prevent the identification of even minor errors or near misses, leaving future patients at risk.
A just culture promotes error analysis within a nonpunitive, blame-free framework, focusing on prevention and continuous quality improvement. Building such a culture requires ongoing coaching and attention to detail. Resources are available to help facilities adopt a just culture, use peer learning among radiologists and implement best practices for peer learning.
