COPYRIGHT LICENSE AGREEMENT

I hereby license to the American College of Radiology (ACR) for limited use my copyright educational material that I have produced and submit to ACR’s Medical Student Curriculum. This becomes effective if and when ACR accepts for publication after reviewing my education material content. This license will enable ACR to reproduce, print and publish the education material electronically or in print. ACR will use my only for educational purposes, as well as future research related to the Medical Student Curriculum.

ACR will obtain no license to the case(s) if it rejects my submission. Whether or not ACR accepts my submission, I will retain all patent rights to the case.

I certify that:

a. The content does not infringe any personal or proprietary rights of others.

b. I have informed or will inform the Editor of any proprietary or commercial interest or conflicts of interest that I may have relating directly or indirectly to the educational material subject.
